Streamlining Care for COPD/HF Patients to Reduce Readmissions and Costs

Patients with concurrent chronic obstructive pulmonary disease (COPD) and heart failure (HF) drive up heath-care system costs significantly. The diseases have some symptoms, root causes, and clinical courses in common, making it possible to treat patients in a way that reduces readmissions. In this study, researchers share 10 strategies to do so, including accurate diagnosis, intensive and evidence-based therapies, addressing other comorbidities, and early risk stratification for exacerbations.
